If you Signed Up on line, you will need to bring the following to the dates below:
Original Birth Certificate
A Utility Bill (Water, Gas or Phone) to show proof of residence
and
a completed physical (forms are available in the link provided above)
*** Physicals need to be completed by August 2nd, 
or the Player/Cheerleader cannot participate***

Physical forms can will also be accepted on the Dates Listed below.
